Pericardial constriction: uncommon patterns.
نویسنده
چکیده
C onstrictive pericarditis can be defined as a syndrome (or syndromes) resulting from compression of the heart caused by rigid, thickened, and frequently fused pericardial membranes. This syndrome was known about more than three centuries ago, but more recently its clinical spectrum has changed in two ways. Firstly, from the aetiologic point of view, there has been an increase in the number of cases of constrictive pericarditis secondary to chest radiation and cardiac surgery. In particular, cardiac surgery has emerged as an important cause of constrictive pericarditis, representing up to 18–29% of cases in some series, 2 although its prevalence is only 0.2–0.3% following coronary artery bypass grafting or valvar surgery. Secondly, the haemodynamic spectrum has been expanded with other forms of constriction. In this paper we will first comment on the classical syndrome of chronic constrictive pericarditis, and then describe the other less known syndromes of cardiac constriction. Finally, some aetiological considerations will be discussed.

Occult constrictive pericarditis.
Four cases of occult pericardial constriction are presented. This condition is not uncommon, but needs high index of suspicion. Integration of detailed echocardiographic evaluation in a given patient with diseases known to cause pericardial involvement can clinch the diagnosis.
متن کاملEffusive-constrictive pericarditis.
BACKGROUND Effusive-constrictive pericarditis is an uncommon pericardial syndrome characterized by concomitant tamponade, caused by tense pericardial effusion, and constriction, caused by the visceral pericardium. We conducted a prospective study of its clinical evolution and management. METHODS From 1986 through 2001, all patients with effusive-constrictive pericarditis were prospectively ev...
متن کاملCardiac tamponade and pericardial disorders in connective tissue diseases: case report and literature review.
Pericardial disorders occurring in connective tissue diseases are not uncommon and may present as acute or chronic pericarditis with or without an effusion. In many instances, a diagnosis of pericardial involvement is not found until autopsy. Echocardiography and other currently employed radiographic techniques have enhanced the ability to make a diagnosis. Approximate frequencies of common con...
متن کاملMetastatic signet ring adenocarcinoma: an unusual cause of cardiac constriction.
Pericardial constriction secondary to metastatic adenocarcinoma is exceedingly rare. We present the first recorded case of pericardial constriction secondary to metastatic signet-ring mucinous adenocarcinoma diagnosed by echocardiography. The cornerstones of echocardiographic diagnosis of constriction are the following: interventricular septal bounce phasic with respiration, M-mode recordings o...
متن کاملConstrictive pericarditis
Constrictive pericarditis — Constrictive pericarditis is the result of scarring and consequent loss of the normal elasticity of the pericardial sac. Pericardial constriction is typically chronic, but variants include subacute, transient, and occult constriction. Pathologically, this results in chronic inflammation and, frequently, calcification. Grossly, the pericardium is considerably thicker ...
